If the mouthwash has only been spilled recently, start by blotting away any excess liquid from cotton twill. Take a clean cloth and dab the stain, being careful not to rub it in further. This should remove much of the liquid and prevent it from penetrating deeper into fabric.
Once excess mouthwash is gone, it's time to tackle the stain that remains. Liquid vinegar is a great natural cleanser and will help to break down and remove any remaining mouthwash particles. Take a damp cleaning cloth and dip it in some white vinegar. Gently blot the stain with the vinegar-soaked cloth, and allow it to sit for several minutes before moving on to the next step.
When the stain has been adequately treated with vinegar, you can move on to washing and drying the fabric. For cotton twill, use cold water and a mild detergent. Once the fabric is throughly washed, it's important to dry it completely. Use either a tumble dryer or air dry to ensure fabric is completely dry before moving it.
